We have exciting opportunities in the Community Teams in Wiltshire Health and Care as we expand our Health Care Support Worker workforce. The Health Care Support Worker role plays a vital part in our community teams, supporting people in their own homes. You will be based in a core community team and will be rostered to support across both the community teams and our new NHS@home (hospital at home) service.

As a health care support worker, you will work alongside our Consultant Practitioners, Advanced Clinical Practitioners, Community Nurses and Community Therapists, functioning as part of a multidisciplinary team supporting the prevention of hospital admissions and assisting people to manage their long-term conditions, enabling them to remain at home.
